Hypertension remains a major public health problem worldwide and requires effective self-management to prevent complications and improve quality of life. However, conventional health education methods are often considered less effective in supporting independent learning among hypertensive patients. This study aimed to develop and evaluate the effectiveness of a digital pocketbook-based hypertension education media to improve self-management among hypertensive patients. This study introduces a practical digital pocketbook integrating visual education and nurse-led self-management support in primary healthcare settings. This study used a pre-experimental one-group pretest–posttest design involving 35 hypertensive patients at a primary healthcare center in Central Java, Indonesia. The intervention involved digital pocketbook-based education combined with nurse-led educational support for four weeks. Data were collected using a hypertension self-management questionnaire and analyzed using paired statistical testing. The results showed significant improvements in participant self-management behavior after the intervention. Participants also demonstrated better medication adherence, dietary management, and self-monitoring behavior. The study concluded that digital pocketbook-based hypertension education media effectively improved self-management among hypertensive patients and may serve as an innovative educational strategy in primary healthcare services
